Set up a Payment Plan
Payment plans can be set up electronically (preferable) or through the submission of a completed Form 27D done via the ShareFile link below.
To request an extended payment plan with a hardship request, please complete the Payment and Authorization Agreement (Form 27D) and the Financial Hardship Request for Individuals (Form FHRIND-NE). This request will also need to have the following documents to accompany the Form FHRIND-NE:
- two months most recent bank statements
- two most recent pay stubs (if applicable)
